Get started by completing our online referral.
Or call our office to sign up with our Patient Portal to complete most intake paperwork.
Contact your child’s pediatrician about faxing us a referral. We recommend having them complete our form when sending pertinent health records.
We send our evaluations and plans of care to your provider for sign off.
We will NOT schedule therapy until after our plan of care is completed, signed by PCP, and returned.
